Modafil MD vs Modalert: Which One Fits You?
Modafil MD and Modalert are both 200 mg modafinil tablets made in India and sold as generic versions of Provigil. The active drug and the dose are identical. What differs is the delivery: Modafil MD is a sublingual tablet you let dissolve under your tongue, while Modalert is a classic tablet you swallow. That small difference drives most of the reasons people pick one over the other.
This comparison uses the FDA Provigil label for drug facts and Afinil's two product pages (checked October 22, 2026) for brand details and prices. We haven't run our own tests on either tablet.
The short answer
- Pick Modalert if you're happy to swallow a pill, want the most widely known Indian modafinil brand, and prefer a standard tablet whose behavior matches the label data.
- Pick Modafil MD if you dislike swallowing pills, or want to see whether a sublingual tablet feels quicker for you.
Neither is "stronger". Both deliver 200 mg of modafinil.

What "sublingual" changes
A sublingual tablet dissolves under the tongue. The idea is that some of the drug is absorbed through the lining of the mouth instead of waiting to pass through the stomach. Afinil says that gives Modafil MD a somewhat faster onset and calls it more effective.
Here's the honest caveat: the official Provigil label only describes standard swallowed tablets. On that label, modafinil reaches peak levels in the blood about two to four hours after a dose, and food can delay that by about an hour. We found no published study that compares Modafil MD with a standard tablet like Modalert. So the speed claim may be true for some people, but it isn't backed by data you can read.
Afinil also says Modafil MD gives an intense burst of up to about seven hours, which it describes as about half as long as a swallowed pill. Again, that's the store's description. If it holds for you, it could be a plus (less risk of sleeping badly) or a minus (it fades sooner), depending on your day.
Modalert: the familiar option
Modalert is made by Sun Pharma, one of India's biggest drug companies, and it's one of the best-known modafinil generics sold online. It's a plain tablet, taken with water, so it should follow the timing on the Provigil label closely.
One thing to know: Afinil's Modalert page says Sun Pharma may stop allowing exports of Modalert and Waklert "in the near future" due to legal issues, while still offering both for sale. We can't confirm when or whether that will happen. If you settle on Modalert, it's worth knowing a backup. Afinil points buyers to ModaXL; you can also read our ModaXL vs Modalert comparison.
Price compared
When we checked, both products used Afinil's common price ladder for 200 mg tablets:
Both started at $3.99 per tablet for a 10-pill pack. The price per tablet dropped to $2.99 at 30 pills, $1.99 at 100 and $1.69 at 200, identical for both brands.
Larger packs go lower still. Afinil also advertised a 24% discount for crypto payments and free shipping over a spend threshold (the pages showed both $130 and $180, so check your cart). Because the ladder is the same, price shouldn't decide this one. See our guide to paying with bitcoin at Afinil if you're considering the crypto discount.
How each is taken
Modafil MD: place the tablet under your tongue and let it dissolve. Don't chew it or swallow it whole. Some people find the texture or taste odd at first; Afinil says it's mint-flavored. Our Modafil MD review covers the routine in more detail.
Modalert: swallow it with a glass of water, like any tablet.
For both, the Provigil label dose is 200 mg once a day, in the morning for narcolepsy or obstructive sleep apnea, or about one hour before the start of a shift for shift work disorder. Higher doses didn't show a consistent extra benefit in the label's studies. Taking either one late in the day can keep you awake at night.
Side effects: the same for both
Changing the delivery doesn't change the drug, so expect the same possible side effects from either brand. In the Provigil label's clinical trials, headache came up most often. Next on that list were nausea, nervousness, a runny nose, back pain, diarrhea, anxiety, trouble sleeping, dizziness and indigestion.
The label also flags rarer problems that need fast action. Any rash is a reason to stop and call a doctor, because modafinil has been linked to rare, serious skin reactions. Swelling of the face or throat, new or worsening mental health symptoms, and chest pain or a racing heartbeat all mean the same thing: stop and get help.
Women using hormonal birth control should know that modafinil can weaken it, both while taking the drug and for a month afterwards. And if a dissolving tablet bothers your mouth, that's a simple reason to prefer a swallowed one. See our medical disclaimer.
Which one suits which person?
Modafil MD may suit you if:
- You find tablets hard to swallow.
- You want to see whether a sublingual form feels faster for you.
- A shorter effect window, if Afinil's description holds, fits your day.
Modalert may suit you if:
- You want a standard tablet whose timing matches the official label.
- You prefer a big-name maker.
- You don't care how the pill is taken, only that it's a well-known brand.
A fair way to compare them yourself
If your doctor agrees you can try both, a simple method keeps the comparison honest:
- Order a small pack of each, rather than a large pack of one.
- Take one brand on a normal workday, early in the morning.
- Leave at least a day before trying the other.
- Keep other things the same: sleep the night before, breakfast and caffeine.
- Write down when you first noticed it, how long it lasted, any side effects and how you slept.
After a few rounds each, the better fit is usually obvious. Never take both on the same day; that just doubles the dose.
Checking what arrives
Both brands come in blister strips. When your order arrives, check that the maker named on the strip matches what you ordered (INTAS for Modafil MD, Sun Pharma for Modalert), that the strength reads 200 mg, and that the expiry date is well in the future. If something looks wrong, contact the store before taking anything. Afinil's reshipment policy explains what it covers if an order is lost or damaged.
Legal notes
Modafinil is a Schedule IV controlled substance in the US and needs a prescription, no matter which brand you buy. Federal rules allow a Schedule IV prescription to be refilled at most five times within six months. Afinil labels these products "OTC", but that doesn't change the law where you live. If you want a legal route, our guide to a modafinil prescription from an online doctor explains the options. More in our legal notes.
Verdict
Modafil MD and Modalert are the same drug at the same dose, from two well-known Indian makers, at the same listed price. Choose Modalert if you want a conventional tablet that behaves like the label describes. Choose Modafil MD if you'd rather not swallow pills or want to test the sublingual format. Whichever you pick, start with a small pack, take it early in the day, and talk to your doctor first.
FAQ
Is Modafil MD stronger than Modalert?
No. Both contain 200 mg of modafinil. Afinil claims Modafil MD acts faster because it's taken sublingually, but we found no head-to-head study, and the dose is the same.
Who makes Modafil MD and Modalert?
Afinil lists INTAS Pharma as the maker of Modafil MD and Sun Pharma as the maker of Modalert. Both are Indian generic manufacturers.
How do you take Modafil MD?
It's meant to be placed under the tongue and left to dissolve, rather than swallowed whole. Modalert is a normal tablet swallowed with water.
Which one is cheaper?
When we checked Afinil on October 22, 2026, both used the same listed price ladder, from $3.99 per pill for 10 down to lower prices for bigger packs. Check the cart for current prices.
Does Modafil MD wear off sooner?
Afinil says it gives an intense burst of up to about seven hours. That's the store's claim, not label data. The Provigil label gives modafinil an elimination half-life of about 15 hours for standard tablets.
Is Modalert still available?
Afinil's Modalert page warns that Sun Pharma may stop exporting Modalert, yet the page still sold it when we checked. If supply ends, ModaXL is the alternative Afinil suggests.
Can I switch between them?
Since both are 200 mg modafinil, switching doesn't change the dose. Don't take both on the same day, and talk to your doctor about any change.
Is either legal without a prescription?
In the US modafinil is a Schedule IV controlled substance and prescription-only, whatever the brand. Federal rules allow a Schedule IV prescription to be refilled at most five times within six months.
